The Forms and Surveys services provides for the design of online forms/surveys for agency web sites. Report generation, and customization can also be included.
Service Level Objective (SLO): Forms and surveys have a 99.5% uptime service level objective including planned outages, maintenance windows and unavoidable events.

Availability (Uptime): 99.5%
Eligible: All state agencies.
Standard Support Hours: 8:00AM – 5:00PM, Monday – Friday.
Exceptions:The Service Desk is not available during State Holidays or Service Reduction Days
Response (to incident) Time: Based on priority, between 1 – 2 hours.
Resolution (of incident) Time: Based on priority, between 4 – 16 hours.
Maintenance Windows: Sat/Sun 12:00am – 6:00am (excluding holidays).
Security: Authentication and data security is provided by the associated application.
